The penetration and barrier problems that fail inspections most often — and how to correct them before the AHJ arrives.
Most failed inspections are not caused by exotic code issues. They are caused by the same handful of conditions repeating above ceilings across an entire building. Knowing the pattern lets you correct it once instead of chasing citations floor by floor.
The recurring seven
- Unsealed or partially sealed cable and conduit penetrations through rated walls.
- Firestop material applied without a matching UL-listed system for the assembly and penetrant.
- Overfilled penetrations — more cable in the opening than the tested system allows.
- Head-of-wall joints left open after ceiling grid or ductwork installation.
- Back-to-back electrical boxes in rated walls with no putty pads or listed alternative.
- Rated barriers with no stenciling, so later trades cut through without knowing.
- No documentation tying the installed condition to a listed system number.
Why they keep coming back
Rated barriers are penetrated continuously through a building's life. Every low-voltage run, security retrofit, and HVAC change is another opportunity for an opening to be created and not restored. Without stenciling and a documented barrier management program, the same walls are reopened by trades who never knew they were rated.
Correcting it before the inspection
A full above-ceiling survey with photo-referenced deficiency logging gives you a defensible scope. Correct against listed systems, stencil the barriers, and hand the AHJ documentation instead of explanations. That is the difference between a walkthrough and a re-inspection.
